Our congregation wishes to participate in the Parish and College Together (PACT) education assistance program at Augustana Univeristy for the academic year beginning in August. Our congregation will provide an educational grant in the amount indicated to the following students from our congregation who attend Augustana. Appropriate officials of our congregation have read and understand the Guidelines for Participation under which Augustana will provide matching assistance. Please note - students must enroll full-time at Augustana University to receive matching funds.

Our congregation agrees to submit payment for our educational grants according to the option checked below. We understand that our check is to be made payable to Augustana University PACT and is to be mailed to the Financial Aid Office.
By checking the box below, I certify that congregational funding for the PACT Program is either included in our parish budget, or is the result of fundraising, special offerings or scholarship funds. I certify that no part of the congregational funding resulted from a direct gift from the family members of any student listed on this Agreement.
